Medical Records

McKinley’s Medical Record Department stores your medical records and maintains strict confidentiality of your records at all times. The clinical staff at the Health Center refers to your records for up-to-date information required for your treatment. Check here to learn more about the medical record. 

If you need information from your medical records, you may request that copies be sent directly to you, another person, healthcare facility or agency.

• Your written signature is required for release of all medical information so you may not request release of your records by e-mail or phone.

• In a medical emergency, contact the Medical Record Department directly at (217) 265-0798 for quick response to your request for records.
  

If you are a current or former student and have questions, you may e-mail the Medical Record Department at medrec@mhc.uiuc.edu for assistance with records release. Please allow 3 days for a response to your e-mail. Note that e-mail communication may be viewed by a third party, and therefore lacks confidentiality.
